Fierce Orlando Medical Waiver and Release of Liability and photo waiver: I, the undersigned participant (or parent/guardian if under 18), acknowledge and fully understand that participation in cheerleading, dance, tumbling, or any related physical activities at Fierce Orlando involves inherent risks, including but not limited to, physical injury, illness, or other medical conditions. I certify that the participant is physically fit and capable of participating in all activities. I agree to notify Fierce Orlando of any changes to the participant’s health or medical condition that may affect their ability to participate. In the event of an emergency, I authorize Fierce Orlando staff to obtain medical treatment for the participant as deemed necessary. I understand that every effort will be made to contact me prior to such action. I agree to be responsible for all costs associated with any medical care. I hereby waive, release, and discharge Fierce Orlando, its owners, staff, instructors, facilities conducting practice or events and affiliates from any and all liability for injuries, illnesses, or other claims arising from participation in any program, class, competition, or event.Please read and sign the waiver to participate in Fierce Orlando. Fierce Orlando Photo & Video Release I, the undersigned participant (or parent/guardian if under 18), grant permission to Fierce Orlando, its representatives, and affiliated media outlets to photograph, videotape, and/or record the participant during classes, events, performances, and activities. I understand that these images and recordings may be used for promotional materials, social media, the Fierce Orlando website, press releases, and other marketing or educational purposes. I waive any right to inspect or approve the finished products or any compensation for their use. If I wish to revoke this consent at any time, I understand that I must notify Fierce Orlando in writing.
